COMPLETION OF CLIENT COMPLIANCE DOCUMENTS

By completing and signing the forms provided by Reviva Skin Lab, you acknowledge and agree to the following terms:

• Voluntary Participation: All information requested in the Client Intake Form, Media Consent and Data Privacy Agreement, Session Tracking Confirmation, Medical History and Consent Form, and Aftercare Acknowledgement is provided voluntarily and with full understanding of its purpose.

• Accuracy of Information: You confirm that all details provided are true, accurate, and complete to the best of your knowledge. You agree to inform Reviva Skin Lab of any changes to your medical or personal information that may affect your treatment.

• Confidentiality and Data Protection: Your personal and medical information will be handled in accordance with applicable privacy laws. Reviva Skin Lab commits to maintaining confidentiality and will only use or disclose information as permitted by law or with your written consent.

• Informed Consent: By signing the relevant consent forms, you acknowledge that you have been adequately informed of the nature, risks, benefits, and alternatives of the treatments, and that you have had the opportunity to ask questions and receive satisfactory answers.

• Media Consent (where applicable): If you consent to the use of your photos/videos for marketing or educational purposes, you acknowledge that Reviva Skin Lab may use these materials across its official platforms, with your identity protected unless explicitly permitted.

• Session Tracking and Treatment Records: You agree that your treatment sessions will be documented for safety, monitoring, and professional purposes. You will confirm each session’s completion and any observations related to treatment outcomes.

• Aftercare Responsibility: You accept responsibility for following the prescribed aftercare instructions. Failure to do so may affect the treatment results or pose health risks, for which Reviva Skin Lab cannot be held liable.

• Right to Decline Service: Reviva Skin Lab reserves the right to decline or discontinue service if forms are incomplete, misleading, or if treatment is deemed unsuitable based on disclosed information.

• Agreement to Terms: Signing any of the documents constitutes your full acceptance of these Terms and Conditions and affirms that you have read, understood, and agreed to all provisions stated herein.
